It means someone else has already submitted an article with the same title. You need to change your title to resubmit.

Not sure, but my impression is this: if you have a track record of articles accepted, and better, sales, editors will be a bit more tolerant of a rejection. If you are worried, then you can always delete the article before it is reviewed again. Go over it with a fine tooth comb, and then resubmit. If you haven't had any articles accepted yet, I'd be careful not to get that third rejection.

Yes it takes longer to have an article reviewed if it is not for a request. Support posted a notice several weeks ago saying that review times were taking a little longer due to many submissions. A week sounds fairly typical. I'd give it another few days before thinking about contacting support.

I think you would have been notified had your account been suspended. That would take the form of a rejection of your most recent article, with a remark letting you know that they won't take any more of your work. This is most often due to plagarism, I believe.
