Formatting: Are Subheaders Allowed
Is it acceptable to create scannable content? Are subheaders allowed? Are lists and bullets allowed?

Yes, you can add sub headers, lists and bullets, as long as you also have content below each point to explain each point.

I figure title and sub-headers must be twelve point Times New Roman like the rest of the text, but can we bold format the title and sub-headers?

Sure, you can embolden in your document, although they won't appear like that in your extract.

After a sub-heading, should I use a double space before starting the paragraph, or is a single space acceptable?

I always leave a line space between a subhead and para.

How do you distinguish the article title from the subheadings?
Is it acceptable to embolden both and in addition underline the main heading?

I can't answer for everyone, but this is what I do.
Main heading is in Arial size 14 bold
byline arial size 12 bold
Main text TNR size 12 normal
Subheadings TNR size 12 bold
I never use underlining.
Of course, none of this shows up in the excerpt on the submission page, except the spacing. Hope this helps.
