Clients asking for refunds?

I'm just curious if this happens often, as it just happened to me. I had originally written something for a request and it was purchased by someone other than the original requester. The original requester then asked me to do another article on the same topic since the first one was no longer available for purchase. I did so, being VERY careful not to "plagarize" myself & in fact, put what I thought was an entirely new spin on the topic. The original requester purchased the second article & posted it. Now the first purchaser has asked for a refund (which Constant Content provided to them).
Has anyone else had this happen? Should I have refused the request for another article on the same topic? I was under the impression that it was perfectly fine to use the same research to write multiple articles as long as they were different in tone or spin. I just don't want to waste a lot of time doing work that will be considered "unmarketable".
I also want to clarify that I'm not upset with the folks here at Constant Content about this. I just don't want to get myself in the same predicament again.
