3 Strikes and Out Rule

I have read about this rule but confess that I find it confusing. Does it mean an article can only be submitted 3 times to get accepted and if it is rejected 3 times then you are banned from CC? Or does it mean that you can only have 3 rejections ever. Logic tells me it's the former since prolific authors would have an increased chance of getting banned than others. Can anyone tell me more or point me at the guidance?

Yep, I was kinda freaked when I garnered two rejections in one day. It's good to know that the rule is tempered with a bit of common sense. - Phil
