You, We and Us

I think I got some rejections recently for mixing up you and we in an article and I want some clarity for a current article:

example:
Is there a way to gain better focus and feel a sense of accomplishment in your daily routine? (you)

Often we blame our lack of progress on having too much to do and not enough time to do it (we)

failing to manage our life is often at the core of what holds us back. (our & us)

Can I use all three? It's late and I'm getting mind-boggled.

If you have these doubts, you don't really have to use any of them:
